
Il peut être proposé aux patients présentant des douleurs neuropathiques centrales rebelles (post-AVC, …) des séances de rTMS (résonnance magnétique transcrânienne).
Cette technique de neuromodulation repose sur l’utilisation d’un champ magnétique appliqué sur le cortex à l’aide d’une bobine. Divers paramètres de stimulation sont utilisés, en particulier la fréquence haute ou basse, selon que l’on recherche un effet stimulant ou inhibiteur de zones cérébrales précises impliquées dans la modulation de la douleur. L’intensité de la stimulation est calculée en fonction du seuil moteur qui correspond à l’excitabilité du cortex moteur d’un sujet donné.
Cela se réalise en ambulatoire, sur le CHS Esquirol, en partenariat avec le CHU de Limoges.
